Please go to https://medicalcannabis.utah.gov/resources/evs-access/ to create an account and patient profile with your current information, save, save and submit for certification. Your status bar at the top should be highlighted "awaiting certification". if you already have an UtahID account use that for EVS.

If you get stuck don't worry! We will help at your visit.

Please bring your current valid government issued form of ID and your phone or internet enabled device to the clinic or special event. We will be checking you ID, collecting payment for the visit, and helping you log into your EVS account on your device to make sure you're  ready!

A 5-10 minute verbal interview between you and the medical provider where you will discuss your qualified preexisting conditions, past treatments, and why you feel cannabis would be a good option for you.

The state of Utah requires a $15 processing fee through the EVS website to complete your application. this fee is in addition to the consult cost and must be placed on a debit/credit card. We will walk you through the process before you leave.

Please log into your http://evs.utah.gov/ account before your consultation. For renewals please make sure you are with in 30 days of your expiration date, to reactivate please update any needed information and hit "save and submit for certification" so your account is in "awaiting certification". Please have your account open on your mobile device when you come in.

That's it. If you are renewing your card before the expiration date you will receive an updated PDF version via email the day after your current expiration date. If your card was expired it will come via Email as soon as the state approves it.
